When practical, volunteers should not put themselves in a position in which they are alone with a single child and cannot be observed by others. Allegations or suspicions of child abuse are taken seriously and are reported to police and/or state agencies for investigation.

I have been informed of Waterville Area Habitat for Humanity’s position regarding child abuse, and have read and understand the portions of my Volunteer Application and Agreement titled, Waterville Area Habitat for Humanity Position on the Nationwide Problem of Child Abuse. I understand that in addition to the state mandated checks, Waterville Area Habitat for Humanity/ReStore will also conduct a thorough check of my background.
I understand that allegations or suspicions of child abuse are taken very seriously by Waterville Area Habitat for Humanity/ReStore and will be reported to police and/or state agencies for investigation and that the Waterville Area Habitat for Humanity will fully cooperate with any related investigation and pursue the prosecution of child abusers to the full extent of the law.

I hereby affirm that my answers to questions on this application are true and correct, and that I have not knowingly withheld any fact or circumstances that would, if disclosed, affect my application unfavorably. I understand that any false information submitted in this application is cause for denial of this application or termination of my volunteer services regardless of when or how discovered; and that my service is subject to government regulations and Waterville Area Habitat for Humanity/ReStore’s review and acceptance of proof of minimal age. Minimal age to volunteer at the ReStore is 16 years and 18 years for all other volunteer opportunities.
I hereby waive any right to claim that this request or investigation is an invasion of my privacy, since they are made with my consent and it is in my best interest while being considered for a volunteer position.

I hereby authorize the Waterville Area Habitat for Humanity/Restore and its designated agents and representatives to conduct a comprehensive review of my background to be generated for internship/employment and or volunteer purposes. I understand that the scope of the report may include, but is not limited to the following areas: verification of social security number; current and previous residences; employment history, education background, character references, drug testing, civil and criminal history records from any criminal justice agency in any or all federal, state or county jurisdictions; driving records, birth records, and other public record(s).
I further authorize any individual, company, firm, corporation, or public agency to divulge any and all information, verbal or written, pertaining to me to the Waterville Area Habitat for Humanity/ReStore or its agents. I further authorize the complete release of any records or data pertaining to me which the individual, company, firm, corporation, or public agency may have, to include information or data received from other sources. The Waterville Area Habitat for Humanity/ReStore and its designated agents and representatives shall maintain all information received from this authorization in a confidential manner in order to protect the applicant’s personal information, including, but not limited to, addresses, social security numbers, and dates of birth.
